Medicaid Waiver — Louisiana

ROW Waiver (Residential Options Waiver)

Supports adults with physical disabilities who want to live in the community rather than a nursing facility. If a care team tells you a nursing home is your only option — ROW is the direct answer. Covers personal care, supported living, assistive technology, respite, and more.

Medicaid Waiver — OAAS

Community Choices Waiver (CCW)

Louisiana's primary Medicaid waiver for seniors (65+) and adults ages 21–64 with adult-onset physical disabilities at risk of nursing home placement. Provides an individualized service budget covering personal care, home modifications, assistive technology, home-delivered meals, caregiver respite, and support coordination.

Note: CCW does not provide 24-hour coverage on its own. Additional paid or unpaid supports may be needed.

OAAS HCBS (Home & Community Based Services)

Provides a range of services to help older adults and people with physical disabilities remain living at home and in their community rather than in a nursing facility. Services include personal care, homemaker assistance, adult day care, and respite care.

PACE (Program of All-Inclusive Care for the Elderly)

For individuals 55+ who meet nursing facility level of care criteria. Coordinates the full range of health and personal care services under one interdisciplinary team — including adult day health care, primary care, home health, therapy, prescriptions, and nursing care when necessary.

Housing

Home Modification Programs

Multiple state and federal programs fund ramps, roll-in showers, wider doorways, and other accessibility modifications for people with physical disabilities. Ask your social worker about LAHAP and HUD Section 504 grants.

What is a Support Coordinator?

Once enrolled in a Medicaid waiver program like NOW or ROW, you're assigned a support coordinator — or you can choose your own. This person becomes your advocate inside the system: they meet with you regularly, help identify services, submit requests, and follow up with providers on your behalf.
