SCI Trust Fund Program

Provides direct financial assistance for people with traumatic spinal cord injuries. Covers personal care, assistive technology, adaptive equipment, home modifications, and community support. This is a grant, not a loan. Funding is limited — apply early.

ROW Waiver (Residential Options Waiver)

Supports adults with disabilities who want to live in the community rather than a nursing facility. If a care team tells you a nursing home is your only option — ROW is the direct answer. Covers personal care, supported living, assistive technology, and respite.

Medicaid Waiver — Louisiana

NOW Waiver (New Opportunities Waiver)

Louisiana's primary Medicaid waiver for adults with physical disabilities who were injured at age 21 or younger. Covers personal care attendant (PCA) services — meaning you can hire someone you trust and pay them through the program. Also covers assistive technology and home modifications.

Community Choices Waiver (CCW)

Louisiana's primary Medicaid waiver for adults ages 21–64 with adult-onset physical disabilities at risk of nursing home placement. Provides an individualized service budget covering personal care, home modifications, assistive technology, home-delivered meals, and support coordination.

Provides a range of services to help people with physical disabilities remain living at home and in their community rather than in a nursing facility. Includes personal care, homemaker assistance, adult day care, and respite care.

Office of Aging & Adult Services

PACE (Program of All-Inclusive Care for the Elderly)

Coordinates the full continuum of health care services through one interdisciplinary team. Includes adult day health care, primary care, home health, personal care, physical and occupational therapy, prescriptions, and nursing facility care when necessary. Goal: stay safely in the community as long as possible.

Must be 55+ and meet nursing facility level of care. Available in select parishes including East and West Baton Rouge. Call 1-877-456-1146.

Medicaid — Louisiana

Nursing Facility Services

When care needs exceed what can safely be provided at home, Louisiana Medicaid covers nursing facility placement. Provides 24-hour care including skilled nursing, personal care, therapy, meals, and social programming.

Important: Nursing facility placement is often discussed as the only option — but most people qualify for home and community-based alternatives like CCW, ROW, or PACE. Explore those first.

What is a Support Coordinator?

Once enrolled in a Medicaid waiver program like NOW or ROW, you're assigned a support coordinator — or you can choose your own. This person becomes your advocate inside the system: they meet with you regularly, help identify services, submit requests, and follow up with providers on your behalf.
